The two most common complaints I hear word-for-word:

- “My outer lips (labia majora) look deflated and wrinkled, almost like they aged 15 years overnight.”
Other frequent changes:
- Flattening or hollowing of the mons pubis
- Prominent or elongated-appearing clitoral hood

- New or worsened vaginal dryness (even in women in their 30s)
This constellation of changes has been nicknamed “Ozempic vulva” online – crude, but accurate.
The Mechanism – It’s Mostly Mechanical, Sometimes Hormonal
GLP-1 agonists trigger profound fat loss, including the subcutaneous fat pads in the labia majora and mons pubis that act like built-in pillows. When that volume disappears quickly (30–80 lbs in 6–12 months is common), the skin envelope that was once supported now has nothing underneath it.
The labia minora themselves lose almost no fat, so they suddenly appear relatively longer and more protuberant – exactly like facial hollowing makes the nose or ears look bigger after massive facial weight loss. It’s an optical effect, but it feels very real in the mirror.
Add in the hormonal piece: rapid weight loss, especially from higher starting weights, can lower circulating estradiol and sex-hormone-binding globulin. The result is thinner vulvovaginal tissues and reduced natural lubrication in some patients – a perfect storm that amplifies the visible and sensory changes.
You Did Nothing Wrong
I want to say this clearly: wanting to be healthier or finally fit into clothes you love does not mean you signed up to feel self-conscious during intimacy. This is a predictable physiologic consequence of rapid fat redistribution, not a personal failing.
Solutions That Actually Work in 2025
Non-surgical (great first-line options) – expect minimal improvement
